The individual is responsible for; evaluating for level of care and recommending the most appropriate placement; reviewing and approving appropriate level of care recommendations for CARES staff, along with providing case consultation to staff related to medical conditions/illnesses affecting the clients on an as needed basis. As a subsection, as assigned, this position deals with Preadmission Screening and Resident Review (PASRR), the process mandated by federal regulations and Florida administrative code to ensure all individuals, regardless of payer source, are screened for potential serious mental illness (SMI) and/or intellectual disability (ID) prior to being admitted to a Medicaid certified nursing facility.Duties to include but not limited to:Coordinates and conducts comprehensive assessments of applicants for nursing facility placement and home and community-based services. Inputs assessment data into Client Information & Registration Tracking System (CIRTS). Evaluates for level of care and recommends the most appropriate placement; participates as a member of an interdisciplinary staffing team for Level of Care determination.Maintains accurate and up-to-date medical information on clients through regular documentation of activities; ensures timely and accurate data collection, reporting and follow-up with individuals in accordance with established CARES policies, procedures and regulations, as directed in the CARES Handbook.Receives Level I screens from qualified entities/persons in accordance with state and federal regulations, for adults requesting admission to a Medicaid certified nursing facility, regardless of payer source, reviewing the Level I screens for correctness/completion. Returns incorrect/incomplete Level I screen and conducts remediation with delegated entities.As assigned, tracks appropriate timeframes/limits for completion of a Level II evaluation and determination, if applicable,when an individual is admitted to a nursing facility under a provisional admission or hospital discharge those instances, where the Level I screen identifies that the individual has a diagnosis or there is a suspicion of SMI or ID, conducts a comprehensive face-to-face assessment, to be utilized by the Level II screener (KEPRO) in the Level II evaluation. As directed, this would include contacting the individual, scheduling the assessment, gathering all necessary documents, entering all data into the Client Information Registration Tracking System (CIRTS) and submitting the comprehensive assessment to the Level II screening entity.Provides technical assistance to delegated Level I screeners (hospitals and nursing facilities) regarding PASRR activities that are the Department s responsibilities per the Agency for Health Care Administration.Participates in monitoring of placement decisions on cases assigned through an annual recertification process; communicates with community health care providers to ensure appropriate service utilization; maintains updated data in CIRTS on cases assigned in accordance with established CARES policies, procedures and regulations, as directed in the CARES Handbook.Per CARES Policy and Procedure, completes case staffing s, reviewing and approving appropriate level of care recommendations for CARES staff per CFR 441.352c. Provides case consultation to CARES staff related to medical conditions/illnesses affecting the clients. Provides regular in-service training to professional staff in the office on topics appropriate to the aged and disabled pletes routine travel within the assigned unit's planning and service area (Operation of private motor vehicle is required to perform travel.
